As a Physical Therapist for Home Health Care, your key responsibilities will include: Designing and executing prescribed physical therapy treatments for patients dealing with various injuries, illnesses, and functional disabilities, aiming for optimal physical performance. Assessing and screening patients' physical capabilities, documenting findings to aid in developing individualized treatment plans with clear, measurable objectives. Creating and administering rehabilitation programs tailored to the physician's directives and aligned with the goals of the patient and the rehabilitation team, adapting strategies as needed for maximum efficacy. Collaborating with physicians and clinical teams to gather comprehensive patient information, which assists in the effective execution and modification of therapy programs and treatment plans. Providing detailed written instructions and care plans to Physical Therapy Assistants and Home Health Aides, overseeing their performance and ensuring proper documentation is completed. Meticulously documenting patient care observations, interventions, and evaluations in a timely manner, including verbal directives from physicians to ensure prompt implementation. Updating the patient's physician, Clinical Manager, clinical staff, and family on patient progress, providing necessary evaluations, treatment plans, progress reports, and discharge summaries as required. Utilize your expertise to create a lasting impact! Required Qualifications: Degree from an APTA-approved, accredited Physical Therapy Program. A minimum of one year of relevant physical therapy experience is preferred. Current and unrestricted Physical Therapy license. Current CPR certification. Outstanding organizational and communication skills. Compensation: Pay per visit/unit: $54.00 - $76.00. Annual base pay ranges from $85,400 to $117,500. Work Schedule: 40 hours per week. Compensation Transparency: The pay scale is a good faith estimate for full-time (40 hours weekly) positions at the time of posting. Compensation may vary based on location, individual skills, experience, education, and certifications. Annual salary range: $93,000 - $128,000.
