Anticipated End Date: 2026-05-01 Position Title: Field Advanced Practice Provider Job Description: Field Advanced Practice Palliative Care Provider (NP or PA) Location: Columbus, OH area. This dynamic field-based role allows you to deliver essential palliative care directly in patients' homes, including board and care settings, as well as independent living facilities. You will also have the opportunity to provide virtual support for patients' palliative care needs. This approach emphasizes flexibility, responsiveness to client needs, and hands-on engagement. Please note that candidates must be within a reasonable commuting distance to be considered for this position, in accordance with company policy. Your impact will include: Delivering compassionate palliative care services within patients’ homes. Developing and executing individualized clinical care plans for adult patients with complex health conditions. Conducting thorough assessments, including history, physical exams, and diagnostics, and creating treatment plans aligned with patient goals. Addressing both physical and psychosocial needs of patients and families with support from our inter-disciplinary team. Educating patients and families about medication management, illness progression, nutrition, and preventive strategies. Maintaining communication with clinical team members and other healthcare providers to ensure comprehensive care and resource coordination. Documenting patient interactions and care plans accurately in compliance with state regulations using Epic EMR. Participating in ongoing education to maintain licensure. Prescribing medications as per state regulations. Minimum Requirements: Master's degree in Nursing or Physician Assistant Studies (MPAS or MSPAS). Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Ohio. DEA registration or eligibility. CPR certification. Completion of a Tuberculosis test is required for this position. The role may require some travel. Preferred Skills, Capabilities, and Experience: MPAS or MSPAS degree is highly preferred for PAs. Minimum of 2 years of experience in home-based services, complex care, hospice, or palliative care. Experience in bedside hospital care, oncology, emergency room, or ICU settings is highly valued. Proficiency with Electronic Medical Records (EMR, specifically Epic) is a plus. Bilingual in English and Spanish is preferred. Salary Information: The salary range for this position is $107,680.00 to $134,600.00 based on experience and qualifications.
