We are looking for a dedicated Afterhours RN Care Manager to deliver high-quality hospice care during evenings, weekends, and on-call hours. This critical role focuses on ensuring continuity of care and managing clinical services for our patients and their families during non-business hours. About the Role As an Afterhours RN Care Manager, you will assess and coordinate the needs of patients and families, ensuring they receive comprehensive care and support after regular office hours. Your responsibilities will include conducting routine visits, admissions, and urgent after-hours calls as outlined in the patient's care plan. You will work collaboratively with the Interdisciplinary Group (IDG) to promote effective communication among physicians, families, and hospice staff. This position also involves clinical administrative duties, supporting workflow audits, and ensuring compliance with regulations. Weekend Hours: Friday 5pm - Monday 5pm Essential Functions Deliver hospice care services to eligible patients and families effectively. Provide direct in-person patient care during after-hours, including assessments, admissions, and end-of-life care. Conduct assessments of physical, emotional, psychosocial, and spiritual needs. Support the IDG process through regular chart reviews and updates. Collaborate with physicians to develop and revise the Hospice Plan of Care. Educate families on disease progression, symptom management, and medication. Communicate urgent changes in patient conditions to healthcare teams promptly. Maintain accurate clinical documentation and records. Assist in scheduling modifications and triage tasks as required. Support administrative tasks, ensuring documentation compliance and readiness for audits. Promote infection control standards and safety precautions consistently. Lead and mentor field staff as needed. Engage in team meetings, after-hours updates, and mandatory training sessions. Enhance your expertise in pain management and symptom control. Uphold the core values of compassion and teamwork in your practice. Perform additional duties as necessary. About You Education and Experience Current RN license in the state of employment. Graduated from an accredited nursing program. At least one year of RN experience in acute care, hospice, oncology, or related fields. Compliance with any state-specific requirements. Licenses and Certifications Active RN license. CPR certification (in-person). Valid driver's license and reliable transportation. Skills and Competencies In-depth knowledge of hospice principles and regulations. Adept at conducting comprehensive patient assessments. Excellent communication skills with patients, families, and team members. Strong time management and prioritization abilities. Proficient in EMR systems, Microsoft Office, and mobile technologies. Commitment to maintaining confidentiality and quality outcomes. Leadership capabilities to guide and support field staff. Strong organizational skills for managing compliance tasks.
