Job Title: Home Care Registered Nurse (RN) Certified Home Health Agency (CHHA) Coverage Area: Manhattan Office Location: Bronx, NY Position Summary The Home Care RN is responsible for conducting in-home clinical assessments, developing, implementing, and overseeing individualized patient care plans. This role collaborates closely with patients, families, physicians, and interdisciplinary team members to ensure safe, effective care in the home, improve functional outcomes, and reduce hospital readmissions. Key Responsibilities Conduct comprehensive clinical assessments to determine home care eligibility and safety Develop, implement, and manage patient-centered care plans Collaborate with physicians, patients, families, and interdisciplinary care teams Monitor patient progress and update care plans as needed Provide supervision and oversight of Home Health Aide (HHA) services Participate in interdisciplinary case conferences Ensure accurate and timely clinical documentation using Homecare Homebase (HCHB) Maintain compliance with CHHA, state, and federal regulations Work Schedule & Expectations 5–6 patient visits per day Training: First 2 weeks include in-office orientation and field shadowing After training, office visits are limited to in-services or supply needs Qualifications Active New York State RN License and current registration Associate Degree in Nursing required; Bachelor’s Degree preferred Minimum 2 years of experience in home care patient assessments and care planning Minimum 1 year of supervisory experience in a hospital or home care setting PRI assessment experience preferred Proficiency in MS Office and electronic documentation systems Experience with Homecare Homebase (HCHB) strongly preferred Valid driver’s license Bilingual skills a plus (not required) Compensation & Benefits Annual Salary: $105,000 – $110,000 Comprehensive benefits package including: Health insurance Retirement plan Paid time off (PTO) Professional development opportunities Apply now or send your updated resume to be considered for immediate interviews.
