University Home Care Inc is looking for a compassionate and dedicated RN Home Health to provide high-quality nursing care to patients in Monroe, MI and the surrounding areas. The ideal candidate will deliver personalized care in patients' homes, ensuring their health and comfort while supporting their recovery and independence. Responsibilities Provide skilled nursing care to patients in their homes, including assessments, treatments, and patient education. Develop and implement individualized care plans based on physician orders and patient needs. Monitor patient progress and communicate updates to the healthcare team and family members. Ensure compliance with all regulatory and agency policies and procedures. Document patient care activities accurately and timely in electronic health records. Collaborate effectively with interdisciplinary team members to ensure comprehensive patient care. Educate patients and caregivers on disease management, medications, and healthy lifestyles. Travel to patient homes in Monroe and surrounding areas to deliver care. Requirements Valid RN license in the state of Michigan. Current BLS/CPR certification. Experience in home health nursing preferred. Strong assessment, communication, and critical thinking skills. Ability to work independently and manage time effectively. Reliable transportation for patient home visits in Monroe and nearby communities. Compassionate, patient-centered approach to care. Benefits Competitive pay rate of $65.00 - $100.00 per visit.
