Description: This position provides, coordinates, and directs the provision of home nursing care according to physician's orders. Care is to be based on agency policies and procedures through the competent application of the nursing process. The nurse is responsible to the Clinical Manager. The nurse directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits and periodic office meetings are required. Reliable transportation, a current driver's license, and liability insurance are mandatory. The average number of patients seen daily is 5-8. Each type of visit and activity holds a different point value. The contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points. Guaranteed hours are not provided. If financial needs arise due to low patient census, the contractor may discuss floating to a nearby clinic for additional hours. Types of cases include Start of Care, Wound Care, and Follow-up Visits. First-timers are not accepted; recent specialty experience is a requirement. Weekend requirements vary by location. On-call requirements also vary by location. Must be willing to travel up to 75 miles from home. Mileage reimbursement is provided at the IRS standard rate. Local candidates are accepted at the same rate. The daily productivity expectation is a minimum of 30 points per week. All documentation must be completed within 24 hours of patient visits. Delayed and incomplete documentation impacting patient care may result in a cancellation. There are no guaranteed hours with this contract. Candidates must be open to on-call rotations. Please confirm that the clinician is aware that this is a home health assignment and they will be responsible for driving to patients' homes daily.
