RN - Home Health • About Facility: o Home Health Services - York County o Address: 226 Northpark Dr., Rock Hill, SC, 29730 • General Information: o Average patients/day? 8/day minimum - 9 points must be met per day o Minimum Years of Experience? At least 1 year of Home Health experience o Will you accept a first-time traveler? No • Patient Types/Common Diagnoses: o Adults - only • Chronic meds, Cardiac, Heart Failure, COPD, Wounds, Drains • Required Licensure & Certifications: o BLS, valid DL and insurance • Skills required: o Home Health experience, Homecare Homebase charting experience is required; OASIS-E (assessment tool) experience o Patient assessments, experience with wound care, pleurx, infusion, venipuncture, case management, SOC (Start of Care) Therapy (comfortability to conduct initial patient visits independently, chronic disease management/maintenance, plan of care, medication management, patient/family teaching • Support within the Department: o RNs o Triage - functions as weekend leadership o Leadership • Technology/Equipment: o EMR: Homecare Homebase - experience required o Home Health Oasis assessment tool - experience required o Department specific: IV Pump: CADD - experience preferred; wound vacs • Floating: o N/A - Home health o This . anch sees York and Lancaster counties (South Carolina); could be asked to float into Union or Mecklenburg County but not likely • Weekend-only position could drive up to 100 miles, depending on case load • Orientation (facility and unit): o How many days are provided? Typically, 1 week - very fast orientation period • Scheduling: o Weekend rotation: no weekends, unless posted as a weekend only position (Fri/Sat/Sun) o No call o Holiday Expectations: will work holidays as scheduled o RTO requests: Manager approval o Shift times: • Weekday positions: (8h) 0830-1800 as long as 8 points are met for the day • Weekend only positions: (12h) 0830-2030 as long as 9 points are met for the day • Other notes: o Scrub Color: Navy
