Reason for Coverage: Resignation of Hematology Oncologist at Practice 30% Hematolgy 70% Oncology Number of Physicians in Practice: 1 remaining physician Hem/Onc only practice Adults only No Radiation Oncology on-site, but connection to Rad Onc team at Loyola University Medical Campus, which is 5 miles from practice. No APP's in practice Documentation System/EMR: epic Estimated Credentialing Timeframe: 3 months Practice Setting: Outpatient Free-Standing Community Cancer Center Average Number of Patients Per Day: 16-20 Trauma Level/Designation (Hospital): Level 2 Requirement description : Board Certified - Required Active IL license - Required at time of name clearAvailability - Please provide at time of name clear - must be available FT Local Candidate - Highly Preferred CERTIFICATION REQUIREMENTS : Board Certified Board Eligible STATE LICENSE REQUIREMENTS : Illinois Weekend Requirements : N/A On Call Requirements : N/A
