St. Joseph's Health, part of Trinity Health, is seeking a full-time, Board Certified or Board Eligible Internal Medicine physician to join its growing Hospitalist team as a Nocturnist at Rome Memorial Hospital in Rome, New York - a well-established, community-centered facility offering an exceptional inpatient practice opportunity with strong subspecialty and intensivist support. Job Description: Provide comprehensive inpatient medical care to a manageable census of approximately 5-10 patients per night, ensuring each patient receives appropriate, timely, and high-quality medical management. Conduct thorough admissions from the emergency department and direct admissions from referring physicians, including detailed histories, physical examinations, and diagnostic assessments. Document clinical encounters accurately and efficiently in the Paragon EMR system, ensuring compliance with hospital and regulatory standards. Monitor and manage acute medical conditions for patients admitted to the hospitalist service, including those in the open ICU, with 24/7 intensivist consultant support for complex or critical cases. Collaborate closely with nursing staff, specialists, case managers, and ancillary teams to coordinate effective patient care plans and ensure safe, efficient patient flow. Arrange and coordinate discharge plans, ensuring all necessary follow-up appointments, medications, and home care needs are appropriately addressed before patient transition. Engage in interdisciplinary rounds and handoff communications to ensure continuity of care between day and night teams. Participate in quality improvement initiatives, performance metrics, and safety programs aligned with St. Joseph's Health and Trinity Health's mission and standards of care. Maintain compliance with hospital policies, medical staff bylaws, and regulatory requirements, including documentation timeliness and quality assurance protocols. Provide mentorship and support to residents, students, or advanced practice providers (if applicable), fostering a collaborative and educational environment. Utilize sound clinical judgment and evidence-based guidelines in all aspects of patient care, demonstrating exceptional professionalism and communication with patients, families, and the healthcare team. Participate in departmental meetings, ongoing training, and CME activities to support professional growth and organizational excellence.
