PRN Certified Occupational Therapy Assistant (COTA)Crystal Cove Post-Acute Lacey, WA PRN / Per Diem | Skilled Nursing Facility Crystal Cove Post Acute is seeking a reliable and compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team on a PRN basis. This role is ideal for a flexible clinician who enjoys working in a fast-paced skilled nursing environment focused on restoring independence and functional outcomes. Position SummaryUnder the direction of a licensed Occupational Therapist, the COTA provides skilled therapy services to residents recovering from illness, injury, or surgery. The COTA plays a key role in helping residents improve activities of daily living (ADLs), safety awareness, and functional mobility. Key ResponsibilitiesImplement treatment plans established by the Occupational Therapist Provide therapeutic interventions focused on ADLs, strengthening, and functional skills Accurately document treatments in EMR according to CMS and state requirements Communicate resident progress and concerns to supervising OT and interdisciplinary team Assist with discharge planning and caregiver education Maintain compliance with facility policies and regulatory standards QualificationsActive Washington State COTA license required Graduate of an accredited Occupational Therapy Assistant program Skilled Nursing Facility or post-acute experience preferred Strong documentation and time-management skills Ability to work collaboratively within an interdisciplinary team Schedule & BenefitsPRN / Per Diem - Flexible scheduling based on census needs Competitive PRN rates (Typical WA SNF range: $35$45/hour DOE) Supportive leadership and team-oriented culture Opportunity for future growth within Crystal Cove Post-Acute About Crystal Cove Post-AcuteCrystal Cove Post-Acute is a skilled nursing and rehabilitation community committed to delivering high-quality, resident-centered care.
