Haven is seeking a Full-Time RN Hospice Case Manager to join our exceptional team in the WEST Valley Territory: Including Glendale, Peoria, AZ, Surprise, GoodYear, AZ and surrounding areas to provide meaningful home hospice visits. Position Responsibilities: Manageable caseloads (1218 patients depending on acuity) Focus on quality over quantity. Collaborate with the interdisciplinary hospice team to develop and implement individualized care plans. Educate patients and families on the hospice philosophy, symptom control, and end-of-life care. Maintain accurate, timely documentation in compliance with company and regulatory standards. No traditional night or weekend on-call!
