Registered Nurse for the licensed agency case manager (LHCSA) is responsible for providing nursing assessments of clients in their homes and over the telephone, providing resources to clients, families and aides in regards to agency and community programs, coordination of services, documentation in medical records in accordance with quality standards, and under the guidance of the clinical operations manager supporting Onondaga, Oneida, and Madison counties. Performance responsibilities and standards: Ensure quality and safe delivery of home health services Conduct nursing assessments and supervision of home health aides Case manage a diverse client caseload Develop a plan of care based on the client assessment to meet their needs and using an intra-agency approach Ensure that documentation meets quality standards and is completed as per agency policy Understand and follow the nurse code of ethics, conduct and standards of practice Communicate and coordinate with internal and external resources to meet client needs Maintain established productivity expectation Provide education to clients, families and aides as needed Assist in immunization clinics when needed Participate in triage and on-call coverage rotation Attend team meeting as required Supervision of para-professional staff relevant to patient care: Supervise home health aides in performing designated tasks related to patient care and per the care plan Communicate appropriate information to the home health aide department Assist with home health aide training program when needed Assist with aide evaluations as directed Comply with home aides of CNY policies and procedures and quality initiatives: Maintain confidentiality Demonstrate knowledge and use of infection control practices Demonstrate knowledge of federal, state, and local government rules and regulations Adhere to quality improvement standards Maintain personnel requirements as current Demonstrate nursing skills competency through periodic evaluations Job qualifications: Current NYS registered professional nurse license Nursing degree from a regionally accredited or approved school of nursing, college or university acceptable to NYSDOH Minimum of 1 year nursing experience preferred Current professional liability insurance Current and valid driver's license, auto insurance and a vehicle which can be used for work Self-directed and excellent organizational skills with the ability to prioritize tasks and work independently Strong verbal and written communication skills with the ability to manage stressful situation, include conflict management Efficient and flexible in problem solving with ability to motivate and educate others Ability to manage a caseload of clients and coordinate care Ability to adapt to changing environments and situations Ability to take direction and maintain compliance with agency policies and procedures Ability to get along with others and work as part of a team Ability to use a computer, variety of software and email systems Physical requirements: Ability to sit or stand at a desk 90% of the day Ability to perform tasks involving physical activity (including but not limited to): pushing/pulling on average 50 lbs with a max of 150 lbs; lifting/carrying on average 25 lbs with a max of 40 lbs; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs. Ability to drive in all weather conditions (rain, sleet, snow, wind). Visual/hearing ability sufficient to comprehend written or verbal communication Ability to use telephone and communicate over the phone Frequently uses a laptop computer to enter orders and obtain product information Bloodborne pathogens exposure category I employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
