Pediatric Clinical Pharmacist Fairview is seeking a Pediatric Clinical Pharmacist to join our inpatient pediatric pharmacy team at UMMC West Bank in Minneapolis, MN. In this hospital-based role, you'll provide direct clinical and distributive pharmacy services to pediatric patients while collaborating closely with physicians, nurses, and interdisciplinary care teams to ensure safe, effective, and evidence-based medication therapy. As a Pediatric Clinical Pharmacist, you will deliver comprehensive pharmaceutical care to hospitalized pediatric patients, supporting optimal outcomes through medication management, safety initiatives, and clinical collaboration. This role combines clinical pharmacy practice, medication distribution, education, and quality improvement in a fast-paced inpatient hospital setting. Schedule & Work Details Hours: 0.8 FTE (64 hours bi-weekly) Shift: Rotating day, evening, and night shifts Weekend Rotation: Every third weekend Practice Setting: Inpatient hospital / academic medical center Responsibilities Clinical & Patient Care Provide pharmaceutical care to pediatric inpatients as part of an interdisciplinary care team Review and verify medication orders for appropriateness, including drug selection, dosing, route, interactions, and formulary compliance Monitor patient therapy and recommend medication adjustments in collaboration with prescribers Provide medication counseling and discharge education for selected patients and families Respond to pediatric code blue emergencies as assigned, assisting with emergency medication preparation and clinical support Medication Safety & Quality Ensure medication safety through identification, documentation, and resolution of drug therapy problems Participate in medication safety initiatives, including Adverse Drug Event (ADE) review and Drug Use Evaluation (DUE) activities Complete medication safety reports and contribute to performance improvement efforts Support formulary initiatives, including IV-to-PO conversions and therapeutic substitutions Pharmacy Operations Coordinate and support drug distribution activities in assigned patient care areas Prepare and dispense medications, including sterile products and IV admixtures, using safe and compliant practices Enter and review medication orders in the pharmacy information system Perform medication storage inspections and ensure regulatory compliance Education & Collaboration Serve as a preceptor for pharmacy students and/or residents during clinical rotations Provide education and training to pharmacists, technicians, nurses, and other healthcare professionals Act as a clinical resource for nursing staff on medication administration and drug therapy questions Foster strong working relationships across departments to support patient-centered care Required Qualifications PharmD or Bachelors in Pharmacy Eligible for MN Pharmacy License Completion of an ASHP-accredited PGY1 residency program Preferred Qualifications Completion of a PGY2 Pediatric Pharmacy Residency or equivalent pediatric experience Why Join Fairview?
