Job Description

As an On-Call Licensed Vocational Nurse (LVN), you will take the helm of our residents' nursing needs, applying our Philosophy of Person-Directed Care in a skilled nursing environment. Your leadership will ensure a safe and healthy atmosphere where residents, staff, and visitors are actively engaged in daily living. You will work closely with care partners in the health center, fully accountable for resident care and satisfaction, while guiding a committed team of caregivers. Please note that your duties may be adjusted based on the discretion of the Director of Nursing Services or their delegate. Compensation: $37.00 - $56.00 per hour, based on experience. This is an on-call position with AM, PM, and NOC shifts available on varying days. To excel in this role, you should have: Current LVN/LPN license in the applicable state IP Certification or willingness to acquire it within 30 days Current CPR certification Preferred prior nursing experience in senior care, skilled nursing, post-acute, or sub-acute settings Your role will involve: Leading the team to ensure compliance with established procedures during your shift Delivering resident care, including medication setup and administration Maintaining open communication with physicians, laboratories, pharmacies, and families of residents Supervising and directing care for residents in assisted living and Residential Plus areas Responding to emergency calls in both residential and assisted living environments Identifying safety hazards in residents' apartments and providing necessary safety guidance Contributing to the Resident Review Committee and engaging in Service Plans to evaluate residents' health status Acting as a nurse in the Wellness Clinic Conducting initial and annual resident assessments, including evaluations for changes in health conditions Documenting health updates, conducting interviews, charting, and taking doctors' orders Promoting and safeguarding the rights of every resident Carrying out other essential duties as needed What you gain: As one of the largest nonprofit operators of senior living communities in the nation, we offer a fulfilling career where you can make a difference in individuals' lives, starting with your own.
