Position Summary: The Registered Nurse (RN) - Home Health is responsible for delivering skilled nursing care to patients in their homes. You will work as part of an interdisciplinary team, collaborating with physicians and families to develop comprehensive care plans tailored to each patient's unique needs. This is a full-time role, with flexibility to work on an as-needed basis depending on organizational needs. Essential Duties and Responsibilities: Ensure patient safety and provide high-quality nursing care. Assess patients’ conditions and implement care plans collaboratively. Provide skilled nursing services such as wound care, medication management, and IV therapy. Educate patients and caregivers about disease management and self-care. Collaborate with healthcare professionals to develop and execute care plans. Document all patient care accurately and in a timely manner. Engage in quality assurance and performance improvement activities. Participate in interdisciplinary team meetings as required. Qualifications: Active RN license in Florida. Minimum of 1 year nursing experience; Home Health experience preferred. Ability to work independently with minimal supervision. Strong critical thinking and clinical skills. Excellent communication and interpersonal abilities. Access to reliable transportation. Flexibility to work as needed. Physical Demands: The RN - Home Health role requires physical activities involving lifting, walking, balancing, and navigating various environments.
