Job DescriptionRegistered Nurse (RN) Care Manager - TravelLocation: Sacramento, CA or Los Angeles, CA (Onsite) Travel: Up to 50% throughout California Position Type: Contract / Travel Assignment Pay Rate: $65 - $81 per hour About the Opportunity Nextaff is partnering with a leading healthcare organization providing comprehensive care to older adults through the Program of All-Inclusive Care for the Elderly (PACE). This program focuses on helping seniors remain independent by providing coordinated medical, social, and supportive services in their homes and community settings. We are seeking an experienced RN Care Manager to support interdisciplinary care teams and deliver high-quality nursing care to medically complex and aging populations across multiple care settings. This role requires flexibility, strong clinical judgment, and a passion for patient-centered care. Position Summary The RN Care Manager is responsible for coordinating and delivering nursing care to program participants in various settings including PACE centers, participant homes, clinics, skilled nursing facilities, and virtual environments. This nurse serves as a key member of the Interdisciplinary Team (IDT) and plays a central role in assessing participant needs, developing care plans, and coordinating care across providers and care environments. Key Responsibilities • Participate in Interdisciplinary Team (IDT) meetings, contributing clinical insights, participant assessments, and care plan recommendations. • Conduct comprehensive face-to-face nursing assessments, including physical, psychosocial, and behavioral health evaluations. • Deliver personalized nursing care in collaboration with medical providers based on individualized care plans. • Administer medications and therapeutic interventions in accordance with physician orders. • Perform clinical procedures within RN scope of practice including: o Wound care (including complex wounds) o IV hydration and therapies o Phlebotomy o Ostomy care • Provide triage and outpatient clinical support, initiating appropriate interventions and escalating care when necessary. • Coordinate care transitions with hospitals, rehabilitation centers, and long-term care facilities. • Educate participants, caregivers, and care teams on treatment plans and health management strategies. • Provide clinical guidance and support to Medical Assistants and other care staff as needed. • Participate in a rotating after-hours call schedule. • Provide short-term support to various program sites experiencing staffing needs, high census, or coverage gaps. • Maintain accurate and timely documentation of all care provided. Travel Requirements • Ability to travel up to 50% throughout California. • Travel and accommodations will be reimbursed. • Travel time will be compensated in accordance with state and federal labor regulations. Required Qualifications • Graduate of an accredited nursing program (BSN preferred). • Active and unencumbered Registered Nurse (RN) license. • Minimum 2 years of clinical experience working with medically complex or geriatric populations. • Experience with wound care, IV therapy, phlebotomy, and ostomy care. • Ability to adapt quickly to multiple clinical environments. • Strong communication and critical thinking skills. • Current CPR/BLS certification (or ability to obtain within 30 days). • Proof of TB screening and immunizations. • Valid California driver's license, reliable transportation, and auto insurance. • Clean DMV driving record.
