As a Healthcare Assistant, you will be crucial in improving the quality of life for our patients who require compassionate care. Your key responsibilities will include: Offering personal care and emotional support to patients, including assistance with bathing, grooming, and dressing to enhance comfort. Assisting patients with mobility tasks such as walking, transferring, and positioning. Guiding patients through prescribed exercises as trained. Helping with toileting needs and maintaining personal hygiene. Planning, preparing, and serving nutritious meals tailored to dietary needs. Supporting patients in self-administering their medications and managing electronic medication reminders. Performing light housekeeping duties, laundry, and running errands as specified in care plans. Documenting patient care and observations accurately using our electronic medical record system. Additionally, you may occasionally drive patients to appointments, forging meaningful connections through our empathetic care approach. Qualifications: A high school diploma or equivalent. Valid certification as a Nursing Assistant (CNA) or successful completion of a Medicare-approved home health aide competency program. Preferred: At least one year of experience in a Medicare-certified home health or hospice environment. Other Requirements: Proficiency in English (reading, writing, speaking). Possess a valid driver's license and maintain an insurable driving record. Working Conditions: This role involves a combination of sitting, standing, and walking for the required hours each day. Ability to lift and assist patients weighing up to 50 pounds. Flexibility to work weekends, evenings, and holidays as scheduled.
