Responsibilities: Provide personalized dementia care, following individual service plans while promoting respect, dignity, and independence for each resident. Assist residents with daily living activities such as bathing, dressing, grooming, and meal participation, encouraging self-sufficiency. Participate in care planning discussions and understand each resident's life story and preferences. Serve meals while promoting healthy eating habits and fluid intake; report any changes in eating patterns. Initiate and lead engagement programs that foster socialization and connection among residents. Maintain accurate and updated records regarding residents' conditions, behaviors, and any observed changes. If permitted by state regulations, assist residents with medications in compliance with policies and procedures. Maintain a clean and safe environment, adhering to established cleaning schedules. Respond promptly to emergency requests for assistance, upholding safety protocols. Monitor residents' locations consistently and report any issues to management. Encourage collaboration and teamwork among co-workers. This job description provides an overview of responsibilities and does not encompass all tasks that may be assigned by a supervisor. Qualifications: Education and Experience: High school diploma or equivalent required, with a preference for related experience with dementia patients. Certifications: Current state certification may be required. Physical Demands: Ability to stand, walk, sit, and perform various physical activities. Ability to lift up to 50 pounds and handle resident care safely. Exposure to various health risks and the necessity of maintaining a safe and hygienic environment.
