Job Summary We are seeking a dedicated Mobile Nurse Practitioner Specialist who is passionate about delivering essential screening, preventive primary care, and medical services to our members in a variety of comfortable, non-clinical settings such as homes, community centers, and nursing facilities. Your role is crucial in helping members achieve their health goals while contributing to a broader strategy aimed at ensuring quality, cost-effective care. Essential Job Duties Provide comprehensive medical care and coordination for diverse patient populations, including adults, women's health, pediatrics, and geriatrics. Perform thorough evaluations, including medical histories and physical exams, to identify care gaps and preventive needs. Address acute and chronic health concerns while assessing medical urgencies. Establish and document appropriate medical diagnoses, coordinating specialty consultations as needed. Order and interpret diagnostic laboratory and radiology tests wisely, even in resource-limited settings. Develop and implement tailored, individualized medical care plans for each patient. Schedule follow-up appointments as necessary and facilitate post-discharge care to minimize hospital readmissions and emergency room visits. Conduct face-to-face visits in various settings, including members' homes and public locations. Engage in community-based clinics to enhance relationships and address health disparities. Manage travel for field visits, which may involve routine driving up to 120 miles daily, including occasional overnight stays for special projects. Maintain active cross-state licenses as required by business needs. Collaborate with fellow nurse practitioners to share best practices and improve operational efficiency. Participate in regional meetings and contribute to achieving team goals. Document medical records promptly in the electronic medical record (EMR) system. Occasionally lift and carry equipment weighing up to 50 lbs. while navigating stairs. Work within the established scope of medical practice in conjunction with a supervising licensed physician as mandated by state laws. Travel locally as required based on state and contractual specifications. Required Qualifications A minimum of 1 year of experience as a nurse practitioner or a combination of relevant education and experience. Active, unrestricted national certification from AANP or ANCC. Current and unrestricted Family Nurse Practitioner (FNP) license in the state of practice. Valid Prescriber Drug Enforcement Agency (DEA) license, as permitted by state regulations. Current Basic Life Support (BLS) certification. Reliable transportation with a valid driver's license and adequate auto insurance for work-related travel. Ability to adapt and work effectively in various environments with diverse populations. Independence and self-motivation to work with minimal supervision. Strong verbal and written communication skills. Proficiency in Microsoft Office and electronic medical record (EMR) systems. Preferred Qualifications Experience in home health, community health, or public health settings as a nurse practitioner. Background in managing chronic conditions in home health environments. Experience working with underserved populations facing healthcare barriers. Skills in immunization and point-of-care testing. Bilingual abilities are a plus.
