Join Our Compassionate Team and Prioritize Health We are looking for a dedicated E-Referral Coordinator for a full-time position, 40 hours/week. Shift Availability: Monday-Friday from 9:30am-6pm ET. Available to work weekend shifts twice a month. Available to work on three holidays a year. As an E-Referral Coordinator, you will play a vital role in the referral process, serving as the first contact point for clients and referral sources, ensuring a smooth continuity of care. Your responsibilities will include data collection, compliance, documentation, and follow-up on initial referrals. Key Responsibilities: Monitor multiple e-referral systems for new referrals and manage alerts and notifications. Check emails for requests to pull referrals from relevant systems, performing follow-ups and managing related communications. Review documentation to assess patient eligibility for services including service area, orders, and insurance. Assist with authorization processes as needed. Track the status of all referrals and provide weekly reports on their progress. Communicate effectively with branches, sales teams, and leadership via email or phone regarding status updates and referral decisions. Manage the ERST log daily to track all referrals and related decision outcomes. Maintain productivity levels in line with departmental standards. Adhere to regulatory compliance guidelines, policies & procedures, and best practices. Qualifications: High School Diploma or equivalent. Typing speed of at least 40 words per minute. Basic math skills to analyze data and generate reports. Strong logical reasoning and decision-making skills. Ability to adhere to strict timelines. Effective communication with all management levels. Aptitude for working with multiple screens/systems with attention to detail. Skilled in time management and ability to thrive in a fast-paced environment. Fluent in reading, writing, and speaking English. Consistent and reliable attendance is required. No travel is expected. Preferred Qualifications: Experience in patient referrals and insurance verification. At least 1 year of customer service or healthcare experience, preferably in home health. Understanding of home health service delivery issues. Familiarity with Medicare enrollment and home health agency guidelines. Associate's degree is a plus. Additional Information: This remote position requires a reliable internet connection with a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Employees in specific states will receive a bi-weekly internet expense allowance. Telephone equipment will be provided for business needs, and a distraction-free workspace is necessary to protect member PHI/HIPAA information. Travel: Some travel to training or meetings may be required at Humana offices. Scheduled Weekly Hours: 40 Compensation: The estimated pay range for this position is $40,000 - $52,300 per year, based on location and individual qualifications.
