Interprets the appropriate information needed to identify each patient's requirements relative to age-specific needs. Provides the care needed by adult and geriatric patient groups. Job Functions: Resident's activities of daily living needs are met as outlined in the plan of care and in accordance with policies and procedures. Assists with hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care, etc. Documentation is completed per protocol. Maintains resident environment and safety as outlined in the plan of care and in accordance with policies and procedures. Responds promptly to call lights and alarms. Provides safety measures and maintains cleanliness of unit. Documents actions per protocol. Resident's nutritional needs are met as outlined in the plan of care and in accordance with policies and procedures. Ensures meal delivery, tray preparation, adaptive equipment, and feeding align with the resident's preferences and choices. Documentation is completed as required. Assures resident's rehabilitative needs are met according to the plan of care. Supports range of motion, ambulation, transfers, and proper body alignment while using assistive devices. Documentation follows established protocols. Reports resident rights, refusals, and concerns to the Charge Nurse in a timely manner. Identifies and reports changes in resident physical, emotional, and psychosocial status promptly. Occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks may involve potential contact with blood, body fluids, or tissues.
