We are currently seeking a dedicated Occupational Therapist for our Home Health and Outpatient services. In this role, you will provide essential occupational therapy services to clients in their homes, adhering to physicians' orders to ensure optimal care and recovery. Key Responsibilities: Assist physicians in evaluating clients' functional levels through diagnostic and prognostic procedures. Conduct thorough assessments of clients' needs, including Outcome and Assessment Information Set (OASIS) assessments. Evaluate clients' significant others and home environments to determine support and instruction needed for effective care. Discuss and assure the appropriateness of physician's orders, making necessary adjustments. Collaborate in the development and implementation of an interdisciplinary care plan that includes Occupational Therapy. Guide clients in therapeutic self-care and activities aimed at improving independence and functionality. Teach clients the proper use of medical devices such as wheelchairs, braces, splints, and prosthetics. Provide education and support to clients and their families regarding the Occupational Therapy program. Set realistic goals based on clients' current capabilities and potential for improvement and conduct periodic re-evaluations. Coordinate outpatient services for any required procedures that cannot be performed at home. Evaluate the effectiveness of Occupational Therapy interventions and modify care plans as necessary. Document clinical notes promptly and update client records within specified timeframes. Maintain regular communication with physicians to discuss client status and obtain further orders as needed. Collaborate with team members, sharing Occupational Therapy techniques that can enhance client care. Supervise Home Health Aides biweekly when Occupational Therapy is the only discipline involved in client care. Oversee the Occupational Therapy Assistant at least once a month. Engage in agency activities, in-services, and team meetings to contribute to professional development. Confirm weekly visit schedules with the Director of Nursing to ensure coordinated agency visits. Participate in discharge planning and complete Occupational Therapy Discharge Summaries as required. Commit to ongoing professional development through education, workshops, and research. Adhere to established agency policies and participate in quality assurance and performance improvement initiatives. Attend scheduled patient care conferences to discuss client progress and care strategies. Complete and submit OASIS assessments and visit documentation in accordance with the agency-defined timelines. Gather comprehensive medical histories relevant to clients' present conditions. Qualifications: Graduation from an Occupational Therapy program accredited by the Council of Medical Education of the A.M.A. and the American Occupational Therapy Association. Membership in the American Occupational Therapy Association is preferred. Current license or registration by the state is required. A minimum of one year of institutional experience is mandatory. At least two years of home care experience and one year of supervisory experience are required (two years preferred).
