Key Responsibilities - RN / Registered Nurse / Home Health Perform in-home assessments and develop individualized care plans Provide skilled nursing care under physician-directed orders Administer wound care, injections, medications, and other treatments Accurately document patient status, care plans, and progress notes Educate patients and families on procedures and health management Collaborate with physicians and care teams to ensure continuity of care Coordinate care and lead clinical efforts as an RN Case Manager Conduct tasks such as specimen collection, pain management, and disease monitoring Ensure accurate ICD-10 coding and documentation for compliance and billing Qualifications - RN / Registered Nurse / RN Case Manager Active RN license in good standing in Indiana Minimum of 1 year RN experience (home health or clinical setting preferred) Strong interest in growing into a Case Manager or RN Case Manager role Skilled in wound care, geriatrics, pediatric care, and behavioral health Ability to work independently with diverse patient populations Strong clinical judgment, communication, and documentation skills Why BrightStar Care of Fort Wayne?
