Join our dedicated team as a Registered Nurse (RN) with Home Health of Western Colorado! This PRN role offers the flexibility you need while allowing you to make a difference in the lives of our clients. Pediatric experience is preferred. Home Health of Western Colorado is proud to be part of Volunteers of America National Services, focusing on providing exceptional housing and healthcare services. Pay: $40.07 - $48.10 per hour, based on experience! Schedule: PRN, requires travel to clients' homes, with an anticipated schedule of 2 days per week as needed and on-call support, including weekend rotations with other RNs. The Registered Nurse (RN) will deliver case management and coordination, ensuring the implementation of professional and supportive services to clients in line with nursing standards as per the Nurse Practice Act. QUALIFICATIONS: Graduate of an accredited nursing program with a current RN license in your state. Current RN license in the state of practice; a Bachelor’s degree is preferred. A minimum of two (2) years of nursing experience in long-term care dementia care or acute care is required; home care experience is a plus. CPR certification is a must. Excellent communication and interpersonal skills. Strong organizational, prioritization, and time management skills. Knowledge of Home Health regulatory and reimbursement standards. Must be at least 21 years old, meet MVR organization standards, and possess a valid driver’s license. Customer service-oriented mindset. Able to follow work assignments effectively. Physical capability to lift and move clients as needed. A true team player. Proficient in using computers and electronic devices for duties like documentation and timekeeping. ESSENTIAL FUNCTIONS: Assess client referrals and determine eligibility and appropriateness for home care services. Conduct thorough assessments based on agency standards for effective home care. Collaborate with healthcare professionals to create comprehensive care plans tailored to client needs. Independently manage emergency home care situations and communicate with physicians as necessary. Provide on-call support for clients as scheduled. Participate in human resource management and ensure adherence to company policies. Promote safety and maintain a secure environment for clients and team members. Attend required training sessions and complete assigned education modules. Your duties will also include maintaining confidentiality and participating in quality improvement initiatives.
