Facility Type: Skilled Nursing Schedule: 8 hours/week Flexibility: Choose your days and hours Compensation: $40.00-$45.00 per hour, based on experience Key Responsibilities: Perform thorough nutritional assessments for new admissions and conduct quarterly reviews for high-risk residents. Develop, implement, and monitor personalized nutrition care plans. Provide nutrition education and counseling as needed. Collaborate with an interdisciplinary care team for optimal resident outcomes. Ensure compliance with state and federal regulations, including CMS and DOH standards. Maintain accurate and thorough documentation. Monitor resident weight weekly and monthly to detect changes in nutrition or hydration status. Engage in quality improvement initiatives to enhance nutrition care services. Conduct monthly kitchen sanitation audits and recommend actions to ensure a safe environment. Review and approve menus semi-annually to align with USDA guidelines. Perform additional tasks as assigned within the scope of practice. Experience & Qualifications: Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program. Registered by the Commission on Dietetic Registration. Current state licensure/certification (if required). A minimum of 1 year of clinical experience is preferred. Strong communication skills for effective interaction with patients and healthcare teams. Ability to apply critical thinking in various situations and develop effective improvement plans. Proficiency in general nutrition interventions. Familiarity with state survey processes and nutrition care regulations. Competency in electronic charting systems.
