Two direct hire opportunities are available for inpatient oncology/hematology pharmacists at a leading medical center. These roles focus on the management and oversight of medication regimens for hematology, oncology, gynecologic oncology, and stem cell transplantation (SCT) patients within an inpatient acute care setting. Candidates with a strong foundation in oncology pharmacy are encouraged to apply, with preference given to those who are residency-trained or have equivalent experience. Location: Joliet, IL Schedule: Typical inpatient schedule, including days/evenings with rotating weekends EMR System: EPIC Key Responsibilities: Participate in interdisciplinary rounds to design, implement, and monitor medication regimens for malignant hematology, medical oncology, gynecologic oncology, and SCT patients. Review and reconcile drug therapies to ensure appropriate use, dosing, route, potential interactions, and cost-effectiveness. Collaborate closely with healthcare teams to clarify medication orders and document changes. Provide patient and family counseling on medication adherence, side effects, and discharge planning, particularly for SCT patients. Conduct pharmacokinetic consultations and document dosing for vancomycin, aminoglycosides, and other therapies. Monitor, document, and report adverse drug reactions and medication errors. Verify chemotherapy orders and support nursing staff with medication inquiries. Facilitate chemotherapy desensitizations in critical care settings as needed. Qualifications: Bachelor of Science in Pharmacy Illinois Pharmacist License or eligibility to obtain Post Graduate ASHP PGY2 Pharmacy Residency in Hematology/Oncology preferred Board Certification in Oncology Pharmacy (BCOP) preferred within 18 months of hire BLS Certification Expertise in intravenous admixture services, intravenous drug therapy, chemotherapy, and cellular therapies Strong problem-solving skills and ability to work independently and collaboratively These permanent positions offer the chance to work in a collaborative hospital environment with clinical expertise and interdisciplinary care.
