Job Description

Provide expert physical therapy services in patients' homes as part of a specialized program dedicated to supporting elderly individuals in maintaining their independence and quality of life. This contract opportunity requires visiting clients within Mercer County and North Burlington County, delivering high-quality care through personalized treatment plans. Responsibilities: Perform comprehensive patient evaluations to assess mobility, function, and therapeutic needs Develop and implement individualized treatment plans tailored to each patient's goals Educate patients and caregivers on exercises and home-based care techniques to enhance recovery Collaborate with physicians, nurses, and case managers to coordinate patient care Maintain accurate and timely clinical documentation in compliance with program standards Travel regularly to patients' homes within assigned counties; reliable transportation is essential Qualifications: Graduate of an accredited Physical Therapy program Licensed Physical Therapist in New Jersey Current AHA Basic Life Support (BLS) certification Minimum two years' experience working with adult geriatric populations Proven experience in home health and senior care therapy settings Strong communication, documentation, and problem-solving abilities Reliable transportation for home visits is mandatory Preferred Qualifications: Experience and training in pelvic floor therapy, such as Herman & Wallace Level 1A or equivalent Schedule: Monday through Friday, 8:00 AM to 4:30 PM No weekend or on-call duties required The role involves regular standing, walking, driving, and occasional lifting (up to 50 lbs.), bending, and reaching. Location Overview: Mercer County and North Burlington County offer a blend of suburban comfort and community resources, ideal for healthcare professionals looking to make a meaningful impact in home health care. These areas feature accessible roadways and established neighborhoods, making for manageable travel between client homes.
