Two direct hire opportunities are available for Oncology/Hematology inpatient pharmacists at a medical center in Des Plaines, IL. These roles require a commitment to patient care in an acute inpatient setting with a focus on hematology, oncology, gynecologic oncology, and stem cell transplant (SCT) patients. The facility utilizes EPIC for electronic medical records and offers a typical inpatient schedule with day and evening shifts and rotating weekends. Key Responsibilities: Participate in interdisciplinary rounds daily to assist with medication regimen design and monitoring. Review drug therapies for appropriateness, including dose, form, drug interactions, and cost-effectiveness. Reconcile home medications upon patient admission. Collaborate with prescribers and healthcare teams to clarify medication orders and document changes. Provide counseling on medication regimens, adherence, and potential adverse effects to patients and caregivers. Conduct pharmacokinetic consultations and document dosing adjustments as needed. Monitor and report adverse drug reactions and medication errors. Review and verify chemotherapy orders for oncology and SCT patients. Support nursing staff with medication-related inquiries. Facilitate chemotherapy desensitization procedures in critical care settings as required. Desired Qualifications: Bachelor of Science in Pharmacy. Illinois Pharmacist License or eligibility to obtain one. Postgraduate ASHP Year Two Pharmacy Residency in Hematology/Oncology is highly preferred. Board Certification in Oncology Pharmacy (BCOP) prior to or within 18 months of hire. BLS certification. Experience in intravenous admixture services, chemotherapy, and cellular therapies. Strong problem-solving skills with the ability to work independently and collaboratively. Candidates with some oncology experience are encouraged to apply.
