Job Description

A licensed Occupational Therapist (OT) is needed to provide skilled, in-home occupational therapy services to patients in the Palisade, CO area. This position involves delivering evidence-based, patient-centered care that promotes safety, functional independence, and improved quality of life. The OT will work closely with interdisciplinary teams while practicing autonomously within physician orders, payer requirements, and relevant regulations. Key Responsibilities: Conduct occupational therapy visits including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits Develop and implement individualized treatment plans addressing ADLs, IADLs, upper extremity strengthening, fine motor skills, adaptive equipment training, energy conservation, and home safety Educate patients and caregivers on therapy plans, adaptive techniques, and fall prevention strategies Document all visits promptly and accurately in Homecare Homebase (HCHB), ensuring compliance with Medicare and privacy standards Collaborate with RNs, PTs, Speech Therapists, OT Assistants (when applicable), and leadership for coordinated patient care Provide clinical oversight to Occupational Therapy Assistants when assigned and permitted by Colorado law Qualifications: Current, unrestricted Colorado Occupational Therapist license Degree from an accredited Occupational Therapy program Valid driver's license and reliable transportation Ability to work independently in home-based settings Proficiency or willingness to learn Homecare Homebase (HCHB) Preferred qualifications include prior home health experience, familiarity with Medicare documentation, and experience with geriatric or medically complex patients.
