Job Description

A healthcare facility in Cicero, IL is seeking skilled inpatient pharmacists with oncology and hematology expertise for direct hire positions. These roles provide an excellent opportunity for pharmacists passionate about inpatient acute care oncology, including malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant (SCT) patient management. Key Responsibilities: Participate in daily interdisciplinary rounds to develop and monitor medication regimens for oncology patients. Review and verify drug therapies ensuring appropriate use, dosing, interactions, and cost-effectiveness. Reconcile patients' home medications upon admission. Collaborate with prescribers and healthcare teams on medication order clarifications and document all adjustments. Counsel patients and families on medication adherence and side effects, especially prior to discharge of allogeneic SCT patients. Provide pharmacokinetic consultations, including dose modifications for vancomycin and aminoglycosides. Monitor, document, and report adverse drug reactions and medication errors. Review chemotherapy orders and assist nursing staff with medication-related inquiries. Conduct order verifications and support medication transitions from intravenous to oral where appropriate. Support chemotherapy desensitizations in critical care settings. Provide evening and weekend coverage as needed. Qualifications and Desired Experience: Bachelor of Science in Pharmacy. Current Illinois Pharmacist License or eligibility to obtain licensure. Postgraduate ASHP PGY-2 Pharmacy Residency in Hematology/Oncology highly preferred. Board of Pharmaceutical Specialties Oncology Pharmacy Certification (BCOP) prior to or within 18 months of hire. BLS Certification. Strong foundation or experience in oncology pharmacy, intravenous admixture, and chemotherapy protocols. Excellent problem-solving skills and the ability to work independently. Effective collaboration with healthcare teams to ensure patient safety and medication efficacy. Additional Details: Schedule follows a typical inpatient rotation with days, evenings, and rotating weekends. EMR system utilized: EPIC. The facility supports candidates willing to apply for Illinois licensure. This is a direct hire opportunity.
