RN Case Manager

Job Details POSITION SUMMARY - RN Case Manager POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization in an inpatient rehab setting (Adult and Geriatric units). Some local travel to a neighboring hospital may occur. MINIMUM REQUIRED QUALIFICATIONS - CA RN license BLS 1-3 years acute hospital case management experience Clinical background Knowledge of post-acute care, care coordination, case management, business development/marketing. PREFERRED QUALIFICATIONS - Understanding of insurance authorizations and process LENGTH OF ASSIGNMENT - 13 weeks SHIFT / HOURS PER WEEK - M-F 8a-5p with 1 weekend a month. The weekend shift is 4 hours Sat and 4 hours Sunday (they then flex off the following Thursday).
