Job Title: Clinical Pharmacist Reports to: Chief Medical Officer Organization: The Healthcare Connection (THCC) Location: 1401 Steffan Ave. Cincinnati, OH About The HealthCare Connection: Founded in 1967, The HealthCare Connection was Ohio's first Federally Qualified Health Center (FQHC). Our mission is to provide quality, culturally sensitive and accessible primary healthcare services. THCC is proudly recognized as a Level 3 Patient Centered Medical Home (PCMH), the highest level of recognition attainable for quality care. We serve over 20,000 patients annually and operates two primary care locations and 6 school-based health centers, and growing! We provide services in Primary Care, Pediatrics, Infectious Disease, Substance Use, Integrated Behavioral Health, Dental Services, Women's Health, and Pharmacy. Position Summary: The Clinical Pharmacist is a clinical pharmacist role responsible for the strategic direction, operational management, regulatory compliance, and clinical excellence of all pharmacy-related programs at The HealthCare Connection (THCC). This position provides both hands-on clinical pharmacy services and organizational leadership, supporting high-quality, patient-centered care across the health center. The Director oversees entity-owned pharmacy operations, contract pharmacy relationships, the 340B Drug Pricing Program, and clinical pharmacy services, while ensuring compliance with HRSA, state, and federal regulations. The role also serves as Chair of the Pharmacy Oversight Committee, providing governance, accountability, and strategic alignment for all pharmacy services. This role is positioned to evolve into a Director-level pharmacy leadership position as organizational growth and program complexity expand This position is a clinical/administrative role, with an expected allocation of approximately 60% clinical practice and 40% administrative responsibilities. Key Responsibilities: Clinical Pharmacy Practice (60%) Provide direct patient care as a clinical pharmacist within scope of practice Collaborative care with medical, behavioral health, and care teams Deliver medication therapy management, chronic disease management, and patient education Clinical quality improvement and outcomes-focused interventions Support value-based care initiatives, including medication adherence, quality metrics, and cost-of-care reduction. Serve as a clinical resource and subject matter expert to interdisciplinary care teams Clinical Pharmacy Leadership & Operations Lead day-to-day clinical pharmacy operations in alignment with THCC's mission and organizational priorities. Oversee entity-owned pharmacy operations, including staffing coordination, workflow optimization, patient safety practices, service delivery standards, and performance monitoring. Support financial stewardship of pharmacy operations through inventory oversight, operational efficiency initiatives, and collaboration with Finance. Collaborate with contract pharmacies to support compliance, service quality, and alignment with 340B program requirements. Support administration of the 340B Drug Pricing Program, including eligibility monitoring, data integrity, compliance tracking, and audit preparation, in collaboration with leadership and compliance partners. Partner with external vendors (e.g., Nuvem) to ensure operational performance and regulatory compliance. Develop, implement, and maintain pharmacy policies, procedures, and standard operating workflows. Leadership, Governance & Reporting Serve as a key clinical pharmacy representative on applicable committees and interdisciplinary leadership groups. Provide pharmacy operational reports, metrics, and recommendations to senior leadership. Support Pharmacy Oversight Committee preparation, documentation, and follow-through (rather than chairing, unless you intentionally want that). Provide supervision, mentorship, and performance management for pharmacy staff. Collaborate cross-functionally with medical, quality, compliance, and finance leaders to strengthen integration of pharmacy services within THCC care delivery. Regulatory Compliance, Risk Management & Audit Readiness Ensure compliance with all applicable HRSA, 340B, DEA, Ohio State Board of Pharmacy, CMS, and federal/state pharmacy regulations. Lead and support internal and external audits, including HRSA/340B, DEA, and state board audits. Maintain robust internal controls, documentation, and monitoring systems to ensure program integrity and transparency. Partner with compliance and finance teams to develop, implement, and monitor corrective action plans. Serve as the organizational subject matter expert on pharmacy compliance, controlled substances, and medication safety. Qualifications: Required Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy. Active, unrestricted Pharmacist License in the state of Ohio 2 years of clinical pharmacy practice experience Preferred 2 years of retail or ambulatory pharmacy experience 340B Apexus Certified Expert (ACE) or equivalent credential Active Drug Enforcement Administration (DEA) license.
