Physical Therapist - PRNLocation: Fairlane Gardens Nursing and Rehab - Reading, PA Revital Rehab Services is seeking a Physical Therapist (PRN) to provide rehabilitation services across one or more of our service locations in Reading, PA. In this role, you will assess residents' needs, develop individualized therapy programs, and deliver care that supports rehabilitation, independence, and improved quality of life. Shift Available PRN (as needed) Responsibilities Provide high-quality physical therapy care within the PT scope of practice, following established standards, departmental procedures, and productivity expectations. Assist in evaluations and develop individualized treatment plans, including target lists, short-term goals, and long-term goals. Collaborate with interdisciplinary teams to support each resident's rehabilitation and overall well-being.
