A direct hire opportunity is available for an experienced Clinical Pharmacist specializing in Hematology/Oncology, with expertise in CARTI and Bone Marrow Transplant care. This role, based in Madera, CA, supports an award-winning pediatric hospital near Fresno and offers the chance to contribute to medication safety and best practices within a dynamic inpatient hospital setting. The Pharmacist will report to the Director of Pharmacy and collaborate closely with pharmacy leadership and hospital medical staff to ensure medication management aligns with regulatory standards and promotes patient safety. A key function involves leading practice evaluation, fostering evidence-based improvements, updating policies, and providing critical education on safety and compliance. This position advocates for patients through active participation in the Medication Error Reporting Program (MERP) and utilizes the EPIC electronic medical records system. Key Responsibilities: Oversee medication management to maintain safety and regulatory compliance. Lead continuous practice evaluation and change initiatives based on current evidence and data. Support pharmacy staff through education on patient safety and regulatory topics. Collaborate with hospital disciplines to enhance medication use processes. Act as the patient advocate within medication safety programs. Maintain up-to-date pharmacy policies and procedures. Desired Qualifications: Doctor of Pharmacy degree (required). Licensed Pharmacist in California by the State Board of Pharmacy. Completion of an accredited residency program (preferred). Basic Life Support (BLS) certification within 30 days of hire. 5+ years of hospital pharmacy experience. 2+ years of management experience (preferred). Strong knowledge of local, state, and federal pharmacy regulations, including Joint Commission requirements. Excellent communication, customer service, organizational, analytical, and problem-solving skills. Familiarity with MERP and EPIC EMR system is highly desirable. Specialty board certifications or medication safety certificates a plus. Schedule: Full-time, 40 hours per week. Day shift, five 8-hour shifts.
