Social Services DirectorClassification: Exempt Reports To: Executive Director Department: Social Services Position Summary In keeping with our organization's mission to improve the lives of the Residents we serve, the Social Services Director is responsible for planning, developing, organizing, implementing, evaluating, and directing the Social Services Department in accordance with current federal, state, and local regulations governing long-term care facilities. The Social Services Director ensures that the medically related emotional, psychosocial, and discharge planning needs of Residents are met and maintained on an individualized basis while promoting dignity, independence, and quality of life. Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Resident Assessment & Care Planning Complete comprehensive psychosocial assessments upon admission and as required (MDS, quarterly, annual, and significant change assessments). Develop and maintain individualized care plans addressing emotional, social, cultural, religious, and discharge needs. Participate in and contribute to Interdisciplinary Team (IDT) meetings and care plan conferences. Identify psychosocial risks and implement appropriate interventions in collaboration with the clinical team. Provide ongoing evaluation documentation in accordance with regulatory timelines. Discharge Planning & Transitions Develop, coordinate, and implement comprehensive discharge plans beginning at admission. Collaborate with Residents, families, nursing staff, therapy, and outside agencies to ensure safe and appropriate transitions. Coordinate transfers within the facility and to outside providers, ensuring proper documentation and communication. Resident & Family Support Orient Residents and families to facility services, limitations, and Resident rights. Provide counseling and emotional support to Residents and families as needed. Mediate conflicts between Residents, families, and staff professionally and effectively. Educate families on facility policies and regulatory requirements. Behavioral & Psychosocial Programming Assess Residents for psychosocial group needs and oversee implementation and documentation of group programming. Collaborate with the Activities Department to enhance social engagement opportunities. Monitor Residents with behavioral health diagnoses and coordinate services with psychiatric providers when indicated. Community & Resource Coordination Maintain knowledge of community resources and establish referral networks. Provide linkage to external services including transportation, adaptive equipment, guardianship resources, and financial assistance. Department Leadership & Compliance Supervise Social Worker(s) and Social Services Assistant(s), including performance evaluations and training. Maintain departmental policies and procedures in compliance with state and federal regulations. Ensure timely and accurate documentation within the electronic medical record system. Maintain organized departmental filing systems (electronic and physical). Participate in QAPI initiatives and survey preparedness activities. Attend department head meetings and required in-services. Safety & Professional Conduct Report hazardous conditions and follow established infection control and safety procedures. Maintain confidentiality in accordance with HIPAA regulations. Promote dignity, privacy, and respect in all Resident interactions. Answer call lights promptly when in Resident care areas. Communicate professionally and respectfully with Residents, families, and team members. Supervisory Responsibilities This position directly supervises: Social Worker(s) Social Services Assistant(s) Responsible for hiring recommendations, onboarding, training, coaching, disciplinary action, and performance evaluations within the department. Work Environment This position operates in a skilled nursing/long-term care environment. Regular walking throughout the facility is required. The role routinely uses standard office equipment and electronic health record systems. The employee may be exposed to infectious diseases and must follow infection prevention protocols. Physical Demands Regularly required to talk and hear. Frequent sitting, standing, walking, bending, and light lifting (up to 20 pounds). Ability to navigate resident care areas safely. Position Type & Expected Hours of Work This facility operates 24 hours per day, 365 days per year. Full-time, exempt position. May require occasional evenings, weekends, holidays, and inclement weather coverage. Travel No routine travel is required. Education & ExperienceRequired: Bachelor's degree in social work or related human services field. Minimum two (2) years of experience in a healthcare setting (long-term care preferred). Preferred: Licensed Social Worker (if required by state regulations). Experience with MDS documentation and discharge planning. Knowledge of CMS long-term care regulations. Additional Qualifications Strong interpersonal and conflict resolution skills. Working knowledge of electronic medical record systems. Ability to maintain strict confidentiality. Strong organizational and documentation skills.
