In this role, you'll support clinic staff by assisting with client education, outreach, and basic health services, while helping community members access care and information. This entry-level position offers the opportunity to learn routine tasks under close guidance, with responsibilities expanding as you gain experience. Performs duties which support a diverse range of programs in Public Health which support at-risk clients in program areas including, but not limited to Public Health Field Nursing and Health Education. Serve as a member of a team that integrates services to positively impact the multiple barriers mutual clients have to achieving a safe and healthy lifestyle; works collaboratively with clinicians, case managers, nursing staff and others to maintain continuous communication with clients and to document their progress. Performs visits to clients and their families in their homes, shelters or on the streets to provide information and education; assists professional staff in providing health education and in the implementation of client care plans. Works with parents and providers to assist them in navigating community resources and to provide parents with services which promote health and reduce risk to children including Women, Infants and Children (WIC), car seat, dental clinics, medical resources, and related programs; provides clients with community referrals for services such as housing, legal aid, and childcare. Interviews clients to assess factors for various health and safety risks including HIV, domestic violence and child abuse or neglect; refers clients to appropriate support services or community service agencies; interfaces and coordinates with human services programs as indicated by the situation. Participates in the development of the client care plan for assigned clients; monitors progress and provides information for care plan updates. Promotes appropriate health screening and testing such as prenatal and perinatal health care and/or HIV testing; performs oral HIV antibody testing, obtains blood and urine specimens, and/or conduct various screening procedures, as needed. Advises clients on harm reduction methods related to safe intravenous (IV) drug use and safe sex; distributes needed supplies for these programs compliant with program standards of use. Assists clients, their caregivers, and families in developing healthier lifestyles; provides information regarding ways to reduce the risk of transmission of communicable diseases. Models daily living skills appropriate for the client including financial resource management, housekeeping methods and parenting and childcare skills; provides guidance and support on an independent lifestyle; demonstrates the principles of being a good neighbor to others in the community as it relates to maintaining independent housing; organizes move-in and move-outs. Practices behavioral interventions such as redirection, positive reinforcement, modeling behavior and motivational interviewing; performs crisis intervention at the direction of medical professional staff, as needed. Transports clients to medical appointments, service provider appointments, shopping and to other facilities to meet identified client needs; advocates for clients by assisting them in completing paperwork and guiding/navigating with them through bureaucratic processes. Collects data, maintains records and prepares progress notes and reports in accordance with state and local requirements. Advocates for clients and assists clients and their families to establish and maintain a community support system. Maintains current knowledge of cultural factors, health issues and community resources and services which affect clients. Provides training to less experienced staff and volunteers as assigned. Performs other related duties as assigned. LICENSES AND CERTIFICATIONS Licenses: Possession of a valid California driver's license issued by the Department of Motor Vehicles at the time of appointment. Health Insurance: Medical, dental, vision, prescription drugs, mental health, and chiropractic care for employees and their eligible dependents is available. The portion of the monthly cost that the employees are responsible for depends on their bargaining unit. The County offers a confidential Employee Assistance Program (EAP). The County has an Employee Health Center available to Kings County employees and their dependents enrolled in the County health plan. Deferred Compensation: Participation is voluntary. One plan with several investment options is available. The accumulation of sick leave starts from your first day of employment and sick leave may be requested subsequent to the time it is earned. Retiree Health Insurance: Upon retirement from the County, and depending on bargaining unit agreements, a percentage of accrued sick leave may be converted to be used to pay for retiree health insurance benefits for eligible employee and spouse. Education Reimbursement Program: Full-time, permanent employees are eligible to receive up to $500 for units earned toward an associate's degree, or up to $1,500 for units toward a bachelor's or master's degree. Permanent employees who work at least half-time are eligible. Extra help employees are not eligible.
