Key Responsibilities: Conduct comprehensive evaluations, lead engaging treatment sessions, and provide exemplary speech/language pathology services to patients in their homes or facilities. Collaborate with interdisciplinary healthcare teams to create and regularly update detailed Treatment and Care Plans. Leverage your expertise to evaluate and treat disorders related to speech, voice, language, hearing, and swallowing, diligently working to minimize their impacts. Perform diagnostic assessments and interpret therapeutic treatments, including audiologic screenings. Document and communicate any changes in patients' conditions and their responses to treatment to appropriate supervisors or physicians. Provide guidance and training on alternative communication methods to patients as required. Counsel and educate patients, families, and healthcare staff to foster effective communication practices and enhance understanding. Maintain thorough documentation as necessary, including evaluations, visit summaries, and progress reports in accordance with company standards. Assist in care coordination and take part in discharge planning to ensure seamless transitions for patients. Uphold the highest standards of professionalism and confidentiality during all interactions. Engage in continuous professional development through staff training and pursue ongoing education as mandated by company policies. Your Impact is Significant: Required Qualifications: Must satisfy the educational and experiential qualifications for the Certification of Clinical Competence in Speech Language Pathology or Audiology as outlined by ASHA. At least six months of experience as a Speech Therapist or Speech Language Pathologist. Home Health experience is a considerable advantage. Possess a current and unrestricted professional license. Hold valid CPR certification. Exhibit strong organizational and communication skills. Must have a valid driver's license, auto insurance, and reliable transportation. Compensation and Benefits: Earn between $49.00 to $69.00 per visit/unit, with base pay ranging from $77,200 to $106,200 annually. Actual compensation may vary based on location and individual qualifications.
