RESPONSIBILITIES: Deliver direct nursing care to residents while under the supervision of an RN or as per physician orders. Administer medications and treatments according to orders and facility policy. Monitor vital signs and document any changes in residents' health status. Assist residents with their daily activities, including bathing, dressing, and mobility. Perform basic wound care and manage catheter or ostomy care as allowed by facility policy. Collect specimens for lab tests as ordered. Keep accurate records of care provided and changes in residents' conditions. Interact with residents, families, and the healthcare team about care needs and concerns. Follow infection control protocols and uphold facility policies. QUALIFICATIONS: Valid, unrestricted LPN license in Connecticut. Graduated from an accredited practical or vocational nursing program. Current CPR/BLS certification as required by facility policy. Excellent communication skills and ability to connect with residents and their families. Compassionate, resident-centered approach with strong organizational abilities.
