The Home Therapy Registered Nurse, as per federal and state regulations, provides essential training and ongoing support for patients choosing home dialysis options. This role is dedicated to delivering exceptional care and service to patients and stakeholders alike. Qualified candidates with prior experience in peritoneal dialysis or hemodialysis may be eligible for a sign-on bonus up to $5,000. Please inquire for details. Train patients in-home hemo or peritoneal dialysis, both in-center and occasionally at their homes. We provide training opportunities; backgrounds in in-center hemodialysis, critical care, and home health are valuable. Work Schedule: Monday to Friday, 8 am to 5 pm; modified schedules can be accommodated based on patient needs. Competitive on-call pay available for nursing staff in the on-call rotation. Holidays: Home Training Nurses typically have holiday time off. As a leader in home dialysis, we deliver 17% of dialysis treatments in patients’ homes. Key Responsibilities: Comply with all federal, state, and local healthcare regulations during care delivery. Train patients and/or caregivers in self-care for Peritoneal Dialysis or Hemodialysis, adhering to all regulatory requirements. Conduct home assessments of the patient’s environment and monitor care quality routinely. Effectively communicate patient assessments and ongoing evaluations with the healthcare team. Document all nursing activities in the Electronic Medical Record, ensuring accuracy and continuity of care. Engage in infection control practices and contribute to quality improvement initiatives. Be familiar with emergency protocols and conduct regular reviews of emergency procedures with patients. Maintain proper supply inventory and ensure timely ordering to meet patient needs. Analyze program performance to drive quality improvement in patient care outcomes. Collect and report lab results and communicate critical findings promptly. Conduct monthly medication reviews and administer medications as prescribed. Participate in on-call nursing services on a rotational basis as required. Adapt to changing staffing needs to support patients and the home program effectively. Maintain reliable attendance in this crucial role. Specific to Home Hemodialysis: Coordinate home evaluations for necessary utilities to ensure equipment functionality. Ensure compliance with water sampling and report results according to policy. Qualifications: Current RN license in the applicable state, must maintain good standing. A minimum of 12 months of nursing experience, with at least 3 months in home dialysis modalities (6 months in certain states like Texas). Preferred experience in caring for patients on maintenance dialysis. CPR certification required before patient care assignments. Ability to distinguish primary colors. Complete the Home Therapy Nurse Orientation program and keep skills current annually. Fluent in English, with strong communication skills. Basic computer skills required; proficiency in clinical applications to be achieved within 90 days of hire. Meet any additional practice requirements mandated by the state.
