Responsible for patient assessments, care planning, and coordination of care. Must have relevant experience and required certifications.Responsibilities: Initial assessment of client and family to determine home health needs including physical assessment & history of current & previous illness. Re-evaluates client nursing needs and uses health assessment data to determine nursing diagnosis. Initiates and revises plan of care as client status & needs change. Develops care plan and establishes goals incorporating therapeutic, preventive, rehabilitative actions, including client/family in planning. Administers medications and treatments as prescribed by the physician. Provides healthcare instructions based on assessment and plan of care. Identifies and implements discharge planning needs as part of the care plan development. Acts as Case Manager when assigned by Clinical Supervisor, coordinating care for assigned caseload. Prepares clinical notes and communicates with the primary physician regarding client's needs and changes in condition, reporting at least every sixty days. Ensures arrangements for equipment and necessary services are available. Instructs, supervises, and evaluates home health aide care provided.Requirements: Home Health & Hospice experience required. Must have current Driver's License & Auto Insurance. Required Certification: BLS.
