Healthcare Support Specialist Position Summary: Join our dedicated team at Eventus WholeHealth, where we provide comprehensive, integrated care for vulnerable adults. We deliver our services to adults in skilled nursing and assisted living facilities as well as private residences, supported by a diverse network of healthcare professionals. The Healthcare Support Specialist plays a crucial role in enhancing the effectiveness of clinical care teams by facilitating clinical services alongside their assigned providers. This field-based position may require travel to healthcare facilities located up to 60 miles from your home. Typically, you will enjoy a Monday to Friday day shift routine. Key Responsibilities: Support clinical providers by enabling them to concentrate on patient care. Ensure all patients are seen as medically necessary in accordance with clinical protocols. Act as a liaison between providers and families/staff for effective patient management. Document encounters and time for Chronic Care Management (CCM) and Behavioral Health Integration (BHI) services. Manage patient scheduling and ensure rosters are updated daily when required. Assist with unexpected changes due to acute needs, admissions, or discharges. Prepare clinical documentation for providers and complete basic encounter notes following protocol. Perform medication reconciliation for all scheduled patients. Input essential data into patient encounter notes and ensure proper assignment to providers. Compile and organize clinical paperwork for provider review each day. Facilitate telehealth sessions as needed. Facility Support: Share patient schedules with facility staff as necessary. Assist with triage and documentation as directed by the assigned provider. Ensure all required paperwork is routed appropriately for provider and family communication. Support discharge planning of clinical services when necessary. Check in daily with facility staff regarding patients' status. Patient Support: Maintain patient confidentiality consistently. Document encounters and times for CCM and BHI services. Regularly check communications to ensure all patient needs are met. Inform patients and families about recent visits and new orders. Coordinate transitions of care with the CCM team for improved patient support. Additional Responsibilities: Adhere to Eventus WholeHealth’s policies and procedures, including HIPAA regulations. Follow the company's dress code policy, wearing appropriate scrubs and footwear. Report to your Lead Medical Assistant for further assignments when work duties are completed ahead of schedule. Seek additional training if you need clarity on any assignments. Qualifications/Education: High School Diploma or equivalent is required. A valid driver's license and reliable transportation are a must. Travel of up to 1 hour from home may be necessary. Certified Medical Assistant credential preferred.
