Overview: Position: Admissions Registered Nurse/RN, Hospice Position Type: Part Time Remote/Virtual Position: No Coverage Area: UCONN, Waterbury and Griffin Hospitals/Weekend GIP admissions/visits Salary: $43 - $50/hour Schedule: Every Saturday and Sunday 8am - 8pm/2 12 hour shifts At AccentCare, being a part of our dedicated team means aligning your work with your passion for making a difference in the lives of others. What You Need to Know: If you fulfill the following qualifications, we encourage you to apply: Licensed as a Registered Nurse in the relevant state(s), with preference for an associate degree and CPR certification recommended. Proven nursing experience in hospice, home health, or related fields. Must possess a reliable vehicle, valid driver's license, and appropriate auto insurance. Excellent communication, organizational skills, and proficiency in Microsoft Office Suite and EMR systems. Physical ability to lift patients over 200 pounds (with assistance) and perform various physical duties. Responsibilities: Conduct detailed assessments and collaborate with patients, families, and healthcare teams to create and adjust personalized care plans. Ensure effective communication of care needs and updates among team members and external partners. Educate patients and caregivers, while meticulously documenting care and maintaining clinical records. Participate in on-call rotations, admissions, and interdisciplinary meetings. Supervise and support clinical staff, while engaging in ongoing education and responsibilities within your licensure scope.
