Job Description

Position Summary The Clinical Liaison serves as the primary point of contact between AMG Specialty Care LTACH and referral sources including acute care hospitals, physicians, case managers, and discharge planners. The Clinical Liaison is responsible for evaluating potential patient referrals, educating referral partners on LTACH services, and facilitating appropriate patient admissions. This role combines clinical knowledge with relationship building and strategic outreach to support patient access to specialized long-term acute care services. Essential Duties and Responsibilities Develop and maintain strong relationships with referral sources including hospitals, physicians, case managers, and discharge planners. Evaluate patient referrals to determine appropriateness for LTACH level of care in collaboration with medical and admissions teams. Coordinate with hospital staff, physicians, and internal departments to facilitate patient admissions and transitions of care. Serve as an educational resource regarding LTACH services, admission criteria, and clinical capabilities. Conduct on-site visits with referral partners to review patient cases and identify appropriate candidates for LTACH placement. Monitor and manage referral pipeline to ensure timely review and response to potential admissions. Communicate effectively with internal leadership, nursing staff, and case management regarding admission opportunities. Maintain accurate documentation of referral activity and outreach efforts. Participate in strategic marketing and outreach initiatives to increase awareness of AMG Specialty Care services. Ensure compliance with all regulatory, ethical, and organizational policies. Qualifications Current Registered Nurse (RN), Licensed Vocational Nurse (LVN/LPN), or Respiratory Therapist (RT) license preferred. Minimum of 2–3 years of clinical experience , preferably in critical care, case management, or acute care. Previous LTACH, acute care hospital, or clinical liaison experience preferred. Strong knowledge of hospital discharge planning and post-acute care services. Excellent interpersonal, communication, and relationship-building skills. Ability to work independently while collaborating with interdisciplinary teams. Strong organizational and time-management skills.
