Job Description

Overview As a Care Transitions Coordinator (Clinical Liaison), you will play a vital role in assisting patients as they navigate the post-acute care process. Your primary aim will be to enhance patient outcomes while ensuring referral sources have a positive experience with Enhabit’s services. Responsibilities Develop patient-centered transition plans utilizing evidence-based clinical guidelines, preventative protocols, and metrics to ensure high-quality and efficient post-acute care delivery. Build and maintain strategic relationships with health systems, hospitals, inpatient facilities, physicians, and executive-level stakeholders to represent Enhabit Home Health effectively. Qualifications Graduate of an approved school of nursing or therapy. Licensed to practice in your state. At least two (2) years of relevant field experience is required. Strong understanding of federal, state, and local laws regulating Medicare-certified home health and hospice operations. Basic technology skills, including mobile device operation, are essential. Preferred Education and Experience Registered Nurse (RN) or Physical Therapist (PT) certification is required. Three (3) years of field experience is preferred. Previous experience in home health or healthcare sales is an advantage. Requirements A valid state driver's license is necessary. Must maintain automobile liability insurance as mandated by law. Dependable transportation in good working condition is required. Ability to drive safely in various weather conditions is essential.
