Connect Care. Guide Families. Make Every Day Count. We are looking for a dedicated and experienced Hospice Clinical Coordinator to enhance our team. This pivotal role focuses on streamlining intake and referral processes to ensure seamless support for patients, families, referral sources, and our organization. You will play a vital role in fostering strong relationships, coordinating admissions, and promoting community awareness about our hospice services. Key Responsibilities Respond promptly to referrals from hospitals, home health providers, physicians, case managers, skilled nursing facilities, and other healthcare professionals. Evaluate patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating effectively with physicians and families. Provide education and reassurance to patients and families regarding hospice care, its benefits, and operational processes. Ensure meticulous documentation and coordination of the intake and admission process, adhering to all legal, clinical, and payer requirements. Act as a vital communication link between referral sources and our hospice care team, facilitating smooth transitions and quick responses to needs. Conduct regular visits to hospitals and facilities, maintaining visibility and fostering relationships with discharge planners, case managers, and physicians. Develop strong partnerships with referral sources and discover new outreach opportunities to raise awareness about hospice services. Coverage Area: St. Thomas Rutherford, Stonecrest - all in Rutherford County. About You Education Licensed Nurse with three to five years of hospice experience required. BSN with five or more years of hospice experience preferred. Experience Minimum of three years of hospice experience required. Preferred experience as a discharge planner, case manager, or educator. Licenses and Certifications Current RN license to practice nursing in the state of employment. CPR certification. Valid driver's license and automobile insurance. Skills and Knowledge Thorough understanding of hospice care and the needs of patients and families. Familiarity with community resources and referral networks. Excellent patient assessment and documentation skills. Ability to supervise staff, coordinate services, and effectively manage programs. Outstanding communication and interpersonal skills with sensitivity to diverse backgrounds. Personal Qualities Compassionate and sensitive to the needs of terminally ill patients and their families. Adaptability to manage multiple tasks and shifting priorities. Professional maturity, collaboration, and tact in working with interdisciplinary teams. Ability to educate and influence referral sources, patients, and families. Strong commitment to patient advocacy and delivering quality care.
