Job Summary As a Registered Nurse, you will work in a collaborative environment with strong interdisciplinary teams focused on patient-centered care in a diverse setting. Under the supervision of the Nurse Manager, you will apply evidence-based practices in accordance with applicable policies and procedures. Your knowledge will guide the development of personalized care plans addressing the comprehensive needs of patients and their families. The Cardiac Rehabilitation Nurse will become an integral part of the cardiovascular specialty team, combining roles as a clinician, educator, consultant, and researcher to enhance patient care. Responsibilities: Conduct initial assessments for patients entering the outpatient cardiac rehabilitation program. Identify eligibility and contraindications for participation, along with specific monitoring needs and tailored risk factor modification goals for each patient. Deliver patient education and counseling, documenting personalized treatment plans with measurable goals. Evaluate and document patient responses to exercise therapy, monitoring vital signs and other indicators before, during, and after therapy sessions. Immediately report any significant acute conditions or abnormal trends to supervising physicians. Initiate ACLS measures and emergency care per hospital policy when necessary. Assist with administrative tasks such as patient recruitment, insurance pre-certification, and records management. Stay updated in cardiac rehabilitation through journals, in-services, conferences, and meetings. Qualifications: Current Connecticut State RN License Current BLS/ACLS certifications BSN required; MSN preferred Previous cardiac rehabilitation experience preferred, including knowledge of exercise therapy Two years of recent relevant experience in cardiovascular clinical areas (e.g., Coronary Care, Congestive Heart Failure) preferred Comply with Cardiac Rehab Nurse Competencies Exhibit physical and emotional resilience to meet position demands Desire to collaborate within a comprehensive healthcare team At Hartford HealthCare, we prioritize your career development and growth.
