Per Diem Physical Therapist

Autumn Lake Healthcare is seeking a Physical Therapist PT for a PRN or part-time role. Join our skilled nursing facility and provide exceptional, personalized care that addresses each resident's unique needs. You will motivate patients during treatment sessions to help them function at their best while promoting a healthy lifestyle through improved strength, flexibility, balance, and coordination. As a Physical Therapist PT, you will formulate and implement a comprehensive treatment plan, as well as educate patients, families, and caregivers on diagnoses, progress, and long-term functional outcomes.
