The RN Care Coordinator will work collaboratively across teams to enhance the quality of care for PACE participants, focusing on those with complex needs to decrease hospitalizations and emergency room visits. Key responsibilities include: Coordinating care and ensuring compliance. Managing data and quality initiatives. Upholding professional standards. Engaging with teams and supporting other duties as necessary. Qualifications: Required Education: Bachelor's in Nursing or enrollment in a BSN program within 6 months with completion within 4 years of hire. Previous BSN commitment letters will be accepted. Required Licensure/Certifications: Current RN license with the North Carolina Board of Nursing. eNLC (Enhanced Nurse Licensure Compact) license may apply. BLS Healthcare Provider certification. Valid North Carolina Driver's License. Preferred Licensure/Certifications include national certification in a relevant specialty. Required Experience: Minimum one year of registered nursing experience. Preferred Experience: Background in home health or case management.
