Job Summary: Under the supervision of a Registered Nurse, the LPN will deliver quality and cost-effective direct patient care. You will collaborate with the healthcare team to help patients achieve their care goals. Your role will include providing patient care in a non-acute outpatient environment covering various demographics such as Pediatric, Obstetrical, Gynecologic, Medical, Surgical, ENT, Podiatry, GI, and Orthopedic patients. You will report directly to the Nurse Manager. The LPN's responsibilities will involve: Collecting health information and effectively communicating it to the physician and healthcare team. Implementing the physician's plan of care and equipping patients with necessary health information and education. Coordinating care and utilizing community resources to support patient needs. Executing fundamental nursing tasks and patient care according to established policies and procedures. Qualifications: Education Requirements: Current Connecticut LPN license Minimum Requirements: Previous experience in an outpatient, ambulatory, or physician practice setting. Excellent customer service skills. Strong written and verbal communication skills. Independent problem-solving abilities. Bilingual English/Spanish preferred.
