Key Responsibilities: Support patients and their families during their end-of-life journey. Conduct comprehensive assessments to set goals for symptom management and end-of-life care using effective case management strategies. Work collaboratively within an interdisciplinary team to holistically address patient needs. Build and sustain therapeutic relationships through effective communication with patients and their families. Leverage advanced technology to accurately document skilled services provided. Qualifications: Registered Nurse with a valid CT RN license. Minimum of one year RN experience; Homecare or Hospice experience preferred. Valid driver's license and reliable transportation required. Proficiency with computers is mandatory.
